Licensure Requirements:-

1. Grade 8-12 documents (original and copy)
2. Tempo (original and copy)
3. Medical Certificate ( 85 birr, in health centers)
4. 2 photographs
5. 200 birr

To get Your License From Oromia National Regional State You Need:-

1. Transcript from Grade 8 up-to university ( Including Ministry, Matrik and University Entrance Exam Certificate). Original and copy
2. Medical certificate
3. 3×4 photo Photographs
4. Tempo
5. And 100 Birr Payment

You Will Get Your License From Addis Ababa, Oromia National Regional State Health Bureau Office Near Sarbet near Sarbet pusinik square

1) Medical Certificate
2)Tempo (original and copy)
3)University Entrance Certeficate Copy
4)Matric Certificate original &  Copy
5)Ministry Certificate original &  Copy
6)Grade 9&10 Transcript original & Copy
7)Grade 11&12 Transcript original & copy
8) 3 Similar photos, 3×4 size One for medical Certificate and 2 For License (for orginal and copy )

9) Take All Materials with a claser to Sidama Regional State Health Bureau found near hawassa Piyasa Gabriel Church

10) Then Take The Payment request Paper from the Licensure office

11) Go to Sidama Region Gebiyoch and Finance Bureau ,then Pay 54 Birr and take the receipt back to Sidama Regional State Health Bureau and take Your License

6. Dire Dawa Administration Health Bureau፡-
