Job description
.
Overview:
The admission/discharge nurse is responsible for coordinating the process of admission and discharge for patients, ensuring a smooth and safe transition.
The ideal candidate will have a strong understanding of admission/discharge planning and a passion for helping patients and their families.
Responsibilities:
• Assess patients’ admission/discharge needs, including medical, social, and financial needs
• Develop and implement admission/discharge plans in collaboration with the patient, family, and healthcare providers
• Provide education and counseling to patients and families up on admission and on post-discharge care and follow-up
• Coordinate appointments for follow-up care, such as physical therapy, occupational therapy, and homecare services
• Assist with medication reconciliation and prescription refills
• Provide emotional support to patients and families during the discharge process
• Ensure that all necessary paperwork is completed and that patients have the resources they need for a successful transition home
• Collaborate with other members of the healthcare team to ensure a smooth admission/ discharge process
• Check bills are complete and all fees are paid up on discharge

Job requirements
.
Qualifications and Experience
• BSc in nursing or clinical nurse with valid license
• 2 years of work experience in similar position
• Strong knowledge of admission/discharge planning and case management principles
• Excellent communication and interpersonal skills
• Compassionate and empathetic nature
• Ability to work independently and as part of a team
• Strong organizational and time management skills
• Familiarity with electronic health records (EHRs)
