Job description

 

Amhara Regional Health Bureau will deploy data clerk. The data clerks assigned in the health facilities have following responsibilities,

Leadership Responsibilities:

• Coordinate and support all data collection, registry, analysis and reporting activities within the health facility.

• Coordinate and support all data collection and recording in the chart from other levels of the network, such as community outreach workers, home-based care service providers, and hospital referrals.

• Ensure the availability and utilization of HMIS tools related with PEPFAR HIV program in service departments

• Recording patient data in the register and data entry into the computer or ART database

• Closely work to strengthen the data management system with the health facility HIT, service provider and head

• Ensure data quality on the health facility though training and regular monitoring

• Facilitate data utilization for decision-making in the health facility

• Prepare HIV and HIV related report on monthly, quarterly and annually basis

• Perform other duties that will be given by the health facility

Clinical Coordination and Linkages:

• Ensure that all components of the personalized care plan are adequately documented, e.g., results of laboratory tests, referrals to community care and support or hospitals and other documentation that might flow to support the clients care plan.

• Ensure that patient referrals to hospital services appropriately handled and results obtained well documented, included in the chart and make phone calls to referral facilities to obtain information

Monitoring, Evaluation and Follow-up:

• Record, collect, enter data into appropriate software and generate monthly, quarterly and yearly reports and report to the respective line manager.

• Ensure that all data is accurately recorded in the existing HMIS and monthly, quarterly and yearly reports generated

• Follow the proper implementation of the HMIS

• Work on improving the quality of the data and facilitate the use of the data for decision-making to provide quality health care services.

• Conduct quality checks of the data anti correct the reports (validate the quality of data and information)

• Collect, Compile, Sort and Maintain Patient Medical Records

• Identify trends in healthcare quality, safety, and effectiveness

• Ensure that all data collected flow to the appropriate level, e.g., back to community community-based information system or up to the Woredas.

• The data clerks assigned at the facility will have full responsibility to ensure that health management information system is fulIy implemented and operational at the health center. This ranging from data collection, compilation, analysis and presentation

• Responsible for data entry from different data sources into the HMIS, generation of monthly and cohort reports for analysis and review by health center management and providers to then be used for decision-making.

• Willing To carry out/perform other related duties and/or responsibilities assigned by the immediate supervisor

• Therefore, the data clerks ensure that all client data required by any HIV treatment e.g., ART, TB/HIV, PMTCT, PICT, lab, pharmacy is entered/recorded accurately in appropriate HMIS registers. They are responsible for data entry from different data sources into the HMIS, generation of monthly and cohort reports for analysis and review by health center management and providers to then be used for decision-making. Ensure that effective referrals occur and documentation is complete to all levels, including community care and support services, CBOs, FBOs, and hospital services.

Job requirements

Required Education, training and experience

• level -4 minimum HIT, data base administrator, IT and ICT Computer science and related fields with a minimum of 2 years’ experience

• BSC degree in IT, computer science, ICT,IS, related fields with 1 year experience

• Witnessed qualities of interpersonal relationship skills and teamwork and excellent in speaking/writing/reading of Amharic and English.

• Experience in HIV/AIDS data management is desirable

• Knowledge and skill on HMIS/DHIS2, Smart care is given priority,

• Candidates with direct work experience on this Job position are encouraged.
